These Terms of Use govern access to and use of northcoremedical.com and related public webpages operated by North Core Medical, PLLC. By using the website, you agree to these Terms. If you do not agree, do not use the website.

The website provides general information about North Core Medical, Direct Primary Care, physicians, services, membership options, pricing, and the future clinic. Website content is not individualized medical advice and is not a substitute for evaluation, diagnosis, or treatment by a qualified healthcare professional.
3. No Physician-Patient Relationship
Viewing the website, sending an email, calling the practice, submitting a form, joining a waitlist, or reserving membership interest does not create a physician-patient relationship. A physician-patient relationship is established only after North Core Medical accepts you for care and completes the required enrollment, consent, and clinical processes.

A membership reservation, waitlist submission, or expression of interest is not a guarantee of enrollment, an appointment, availability with a particular physician, or a specific opening date. North Core Medical may establish enrollment limits, eligibility requirements, service areas, physician panels, and other conditions before accepting a membership.
Unless expressly stated otherwise, submitting a reservation does not require payment and does not create a binding membership agreement. Final membership benefits, limitations, fees, cancellation terms, and other conditions will be governed by the membership agreement presented at enrollment.
6. Direct Primary Care Is Not Health Insurance
A Direct Primary Care membership is not health insurance and does not replace comprehensive health insurance. Membership generally covers only the primary-care services specifically described in the applicable membership agreement. Hospital care, emergency services, specialist care, prescriptions, laboratory testing, imaging, vaccines, procedures, and other services may be excluded or separately charged. Members should maintain appropriate insurance or other coverage for services outside the membership.
7. Services, Pricing, and Availability May Change
Website descriptions, anticipated opening dates, appointment availability, physician schedules, membership prices, discounts, included services, and separately billed services are subject to change. Information shown on the website is not a binding offer unless incorporated into a signed agreement.
